What is the difference between All-on-4 and All-on-6?

All-on-4 and All-on-6 are approaches for supporting a fixed full-arch restoration with implants. Implant number is the obvious difference, but anatomy, implant distribution, bite forces, restorative design and maintenance are equally important.

Implant number is only one factor

All-on-4 uses four implants in an arch and All-on-6 uses six. More implants do not automatically make every plan better, and four implants are not suitable for every patient. Position, angulation and available bone determine how support is distributed.

Anatomy and prosthetic design

Posterior anatomy can influence implant positions and may lead to angled posterior implants in some All-on-4 concepts. The restoration must also provide suitable lip support, bite, material thickness and access for cleaning.

How the choice is made

Remaining teeth, bone volume, smile line, functional forces, medical factors and treatment goals are reviewed together. The decision should be based on the complete surgical and restorative plan rather than a package name.
